- Step 4 | TN Immunization CertificateRequired by licensing. Accepted children must upload their certificate by March 1, 2026. Instructions are emailed with acceptance notices.
- Step 5 | Influenza Notification Form
- Step 6 | Teacher Recommendation FormRequired for applicants who will be 3 years old by August 15, 2026.
